Job Summary
The Care Coordinator will play a vital role in coordinating the implementation of the Plan of Care, including a Transition and Crisis Plan, through service delivery with providers and community resources. This position requires a strong understanding of family systems, community resources, and case management principles.
Key Responsibilities
- Coordinating service delivery with providers and community resources
- Cultivating flexible, family-focused, community-based responsive services
- Developing and implementing Transition and Crisis Plans
Requirements
- Bachelor's degree in relevant field preferred
- Minimum three years' experience in children's behavioral health or relevant field
- Completion of high-fidelity wraparound training program
- Experience working with children and families who are experiencing SED, trauma, co-occurring behavioral health disorders
