Role Description

Position Title: Support Specialist
Classification: Non-Exempt
Reports To: Supervisor/Manager of Home or Appropriate Program


Job Summary

The Support Specialist is responsible for fostering a safe, healthy, and supportive environment for individuals in our care, ensuring adherence to their personalized care plans.


Key Responsibilities
  • Promote independence among individuals.
  • Ensure individuals have the necessary resources to maintain their independence.
  • Empower individuals by teaching them essential skills for daily tasks.
  • Safeguard the health and well-being of those served.
  • Assist with medication management and treatment plans as required.
  • Support individuals in maintaining their living spaces, including reporting maintenance issues.
  • Guide individuals in financial management, including budgeting and shopping.
  • Assist with daily living activities such as meal preparation, personal hygiene, and community engagement.
  • Prepare meals and manage household supplies as needed.
  • Document all required information in compliance with relevant policies and procedures.
  • Implement behavioral programs as trained.
  • Support individuals in achieving their personal goals as outlined in their care plans.
  • Identify and address potential behavioral triggers to ensure safety.
  • Provide transportation for individuals as necessary.

Qualifications
  • Minimum age of 18 (non-driver) or 21 (driver).
  • Proficient in reading, writing, and verbal communication in English.
  • Must meet all state agency requirements, including good standing with the state abuse and neglect registry.
  • Basic computer skills are necessary.
  • Previous experience in a support role is preferred.

Physical Requirements
  • Ability to move about to perform job duties.
  • Regularly lift and assist individuals weighing up to 25 pounds.
  • Remain alert to the needs of individuals served.
  • Occasional exposure to health risks.
