About the Role

This RN Case Manager position is a great opportunity for a dedicated and compassionate nurse to make a meaningful difference in the lives of our patients. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that patients receive the best possible care in the comfort of their own homes.

Responsibilities
  • Provide nursing care and coordination to patients in their homes, including lab draws, nebulizer treatments, and other necessary interventions.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Coordinate with healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
  • Monitor patient progress and adjust care plans as needed to ensure the best possible outcomes.
Requirements

To be successful in this role, you will need:

  • A graduate degree from a CCNE or NLN accredited nursing program.
  • A minimum of 2 years of related experience and/or training, or an equivalent combination of education and experience.
  • A valid driver's license and insurability by Avera Health's automobile liability insurance carrier.
