We are seeking a highly skilled and compassionate Physician Assistant/Nurse Practitioner to join our team at Trillium Health NY. As a key member of our healthcare team, you will provide comprehensive care to our patients, working collaboratively with our medical staff to deliver high-quality patient-centered care.
Key Responsibilities- Provide primary and specialty care to patients, including histories and physicals, routine and urgent office visits, and review of laboratory and test results.
- Develop and implement individualized care plans, taking into account patients' unique needs and circumstances.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay current with best practices and advancements in healthcare.
- Contribute to quality improvement initiatives and peer reviews to ensure high-quality patient care.
- Minimum 3 years' experience as a board-certified Nurse Practitioner or Physician Assistant.
- M.S. degree in a National League for Nursing (NLN) accredited school of nursing or M.S. degree in Physician Assistant studies.
- Current licensure in New York State as a Registered Professional Nurse and current certification in New York State as a nurse practitioner or Physician Assistant.
- National certification by the American Nurses' Association (ANA) or other certifying body as a nurse practitioner (adult or family, preferred).
- Experience working in community-based healthcare settings.
- Understanding of Electronic Medical Records (EMRs).
- Excellent written and oral communication skills.
