Job Title: Home Health Physical Therapy Assistant

LHC Group is seeking a skilled Physical Therapy Assistant to join our team in Newington, Connecticut.

About the Role:

As a Physical Therapy Assistant, you will work under the supervision of a Physical Therapist to provide high-quality care to patients in their homes. Your primary responsibilities will include:

  • Providing exercise instruction and patient education to improve patient well-being
  • Following the established plan of care and working within the scope of the physical therapy assistant
  • Observing, recording, and reporting patient responses to treatment and changes in condition
  • Coordinating care with other healthcare team members as needed
  • Instructing patients, families, caregivers, and other healthcare team members in therapy areas within the scope of the physical therapy assistant
Requirements:

To be successful in this role, you will need:

  • A current license, registration, or certification to assist in the practice of physical therapy in the state of practice
  • Excellent communication and interpersonal skills
  • Able to work independently and as part of a team
  • Ability to adapt to changing situations and priorities
