Job Overview

Constellation Health Services is seeking a dedicated Registered Nurse (RN) to join our Hospice Team as a Per Diem RN. This role involves providing skilled nursing care to patients in their homes, focusing on comfort, dignity, and quality of life.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Provide direct patient care, including wound care, medication management, and symptom control
  • Collaborate with the Interdisciplinary Team to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare providers
  • Maintain accurate and timely documentation

Requirements:

  • RN graduate with a current Maine license
  • Hospice care or community field care experience
  • Basic computer skills and ability to navigate electronic medical records
  • Strong communication and interpersonal skills
