Family Preservation and Reunification Coordinator

We are seeking a highly skilled and compassionate Preservation Case Manager to join our team at Samaritas. As a key member of our Family Preservation Team, you will play a critical role in supporting at-risk families and helping them achieve stability and self-sufficiency.
Key Responsibilities- Provide intensive, short-term or long-term in-home crisis intervention and family education to at-risk families, utilizing a strength-based, family-centered approach.
- Conduct comprehensive assessments, develop family goals, and assist families in attaining new skills and resources.
- Collaborate with referral sources, agencies, and individuals to provide advocacy and professional consultation services.
- Participate in team meetings, provide regular updates, and make recommendations about case goals and action steps.
- Assist clients in defining presenting problems, devising methods for resolution, and assessing progress toward treatment goals.
- Bachelor's degree in a human service field or education, or equivalent experience and training.
- Two years' experience working with children and/or families, with a focus on multi-problem families and strength-based approaches.
- Valid driver's license, reliable transportation, and good driving record.
