Key Responsibilities

  • Conduct comprehensive health assessments for patients with acute and chronic conditions.
  • Assess and manage care for patients with minor, episodic illnesses and injuries.
  • Interview and advise patients and families on health and illness prevention.
  • Perform follow-up exams and provide ongoing care and management.
  • Collaborate with patients to determine their physiologic, emotional, and environmental needs and priorities.
  • Provide case management services to assist patients and families with arranging for or referring them to needed services.
  • Manage patient health concerns through telephone triage and consultation as needed.
  • Order diagnostic and laboratory tests, prescribe pharmacological agents, and provide non-pharmacological treatment modalities as needed.
  • Maintain and review patient records, charts, and other pertinent information.
  • Perform patient callbacks, report diagnostic findings, and provide consultation as needed.
  • Participate in call coverage and attend required department meetings.
  • Participate in committees and other activities as requested.

Requirements

  • Graduate of a master's level nurse practitioner program.
  • Registered nurse in the State of Wisconsin.
  • Certification through ANCC or ANA.
  • Certified in the State of Wisconsin as an Advanced Practice Nurse Prescriber or certification eligible.
  • Maintains current CPR certification and ACLS certification desirable.

Working Conditions

  • Hears whispers at 3 feet or less.
  • Hears whispers at 3-8 feet.

Vision Requirements

  • Color discrimination.
  • Near vision (correctable to Jaeger 2 or 20/30 binocular).
  • Distance vision (correctable to Snellen chart 20/30 binocular).
