Job Summary

We are seeking a skilled Registered Nurse to join our team in a Weekend Option role. As a Registered Nurse with our organization, you will have the opportunity to provide high-quality patient care in a home health setting.

Responsibilities
  • Clinical Competence
    • Develop and implement individualized plans of care in collaboration with the physician and other healthcare professionals.
    • Supervise and direct the care provided by home health aides and licensed practical vocational nurses, ensuring adherence to state and federal regulations.
  • Documentation and Care Delivery
    • Provide high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
    • Accurately and timely complete comprehensive assessments, including medication reconciliation.
  • Quality
    • Ensure patients meet home health eligibility and medical necessity guidelines, accurately determining primary focus of care and developing plans within state-specific guidelines.
    • Communicate relevant information effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
  • Professionalism
    • Display a positive attitude, willingness to help patients and the agency, and openness to learning new skills.
    • Participate in all meetings, including case conferences, and adhere to agency policies and procedures.
  • Teamwork
    • Take direction from Clinical Director and Executive Director professionally, completing tasks assigned timely and adhering to agency policies.
    • Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
Requirements
  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation
