We are seeking a compassionate and skilled Senior Care Coordinator to join our team at American Senior Communities. As a key member of our care team, you will be responsible for coordinating the completion of resident assessments, managing the interdisciplinary care plan process, and promoting individualized support and care of residents.

Responsibilities:

  • Coordinate the completion of resident assessments using RAI tools
  • Manage the interdisciplinary care plan process
  • Act as a resident advocate and promote individualized support and care of residents
  • Oversee the management of and information involving resident assessments for case mix reimbursement and transmit MDS reports according to strict guidelines
  • Serve as a member of Quality Improvement committees to enhance positive resident outcomes

Requirements:

  • Graduate of an accredited School of Nursing, preferably BSN
  • Minimum of one year in nursing management in the long-term care industry
  • Two years professional nursing experience in long-term care, acute care, restorative care, or geriatric nursing setting
  • Knowledge of the Resident Assessment Instrument, Interdisciplinary Care planning process, and Minimum Data set 2.0
  • Knowledge of OBRA regulations and familiarity of current state reimbursement guidelines
  • Knowledge of current laws and regulations that influence provision of care and services in nursing facilities
