Job Description

We are seeking a skilled Geriatric Physical Therapist to join our team. As a member of our multidisciplinary team, you will be responsible for developing personalized rehabilitation programs for patients with complex medical conditions.

Responsibilities:
  • Treatment Planning: Discuss treatment options with patients and their families, taking into account their unique needs and goals.
  • Patient Care: Administer treatments to rehabilitate patients, utilizing various physical therapy modalities to promote optimal outcomes.
  • Home Visits: Perform home visits when necessary, ensuring continuity of care and adapting to changing patient needs.
  • Care Coordination: Collaborate with interdisciplinary teams to develop comprehensive care plans and ensure seamless transitions between settings.
