Job Summary

Critical Connection, inc is seeking a skilled Physical Therapist to join our team at a Skilled Nursing Facility in Framingham, Massachusetts.

Job Description

The Physical Therapist will be responsible for creating and executing physical therapy programs tailored to each client's needs, encouraging patient participation in tasks and therapies designed to restore, reinforce, and enhance independence and productivity following illness or injury.

The ideal candidate will have a Master's Degree in Physical Therapy from an accredited program, a Physical Therapy license, and prior experience in Skilled Nursing Facilities or Long-Term Care settings.

Responsibilities
  • Determine appropriate PT, POC & D/C and make adjustments as necessary
  • Develop and implement individualized physical therapy plans
  • Conduct patient evaluations and assessments
  • Provide education and training to patients and caregivers
  • Collaborate with interdisciplinary teams to ensure comprehensive care
