As a Home Care Registered Nurse with HexaQuEST Health, Inc., you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Your expertise will be essential in assessing patient needs, developing and implementing personalized care plans, and providing ongoing supervision to ensure the best possible outcomes.
Key Responsibilities:- Conduct thorough assessments of patients' needs and develop comprehensive care plans in accordance with evidence-based practice, State and Federal regulations, and VNAHSR policies.
- Collaborate with patients, caregivers, and healthcare teams to ensure seamless care coordination and effective communication.
- Implement medical plans of care, nursing interventions, and procedures to ensure safe and effective patient care.
- Stay up-to-date on the latest high-tech skills, including IV use and maintenance, wound VAC, and complex wound care.
- Work collaboratively with LPNs to ensure that patient needs are met and care plans are followed.
- Evaluate patient responses to care and make adjustments as needed to ensure optimal outcomes.
- Maintain accurate and up-to-date documentation of patient care, including OASIS documentation and Agency forms.
- Participate in monthly peer review audits and provide constructive feedback to colleagues.
- Proactively advocate for patient care issues and ensure that overall quality and type of care is sensitive to each patient's unique needs.
- 2 years of experience as a Home Health RN
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Strong problem-solving and critical thinking skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
This is a full-time position with a normal shift schedule, Monday-Friday 8am-4pm, with some on-call or weekend requirements.
