About You:

We are seeking a highly skilled and compassionate Nurse Practitioner to join our HouseCalls team in Lake and Orange counties, FL.

The ideal candidate will have strong clinical skills, excellent communication and interpersonal skills, and the ability to work effectively in a fast-paced environment. As a member of our team, you will provide high-quality patient care, conducting thorough in-home assessments and developing personalized treatment plans to improve patient outcomes and satisfaction.

Responsibilities:

  • Conduct thorough in-home assessments, including reviewing medical history, vital signs, and performing comprehensive physical exams.
  • Develop evidence-based treatment plans and communicate effectively with patients and their primary care physicians.
  • Educate patients on disease management, medication adherence, and healthy lifestyle choices.
  • Maintain accurate records and documentation of patient interactions and outcomes.

Qualifications:

  • A minimum of 1 year of experience as a Nurse Practitioner in a clinical setting.
  • Active NP license and ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care.
  • Ability to work autonomously and as part of a team, providing high-quality patient care in a fast-paced environment.

Compensation:

  • A competitive salary: $120,000 - $150,000 per year, depending on experience.
