Role Responsibilities

The Patient Care Coordinator (PCC) is responsible for delivering high-quality, patient-centered care while coordinating unit functions. Collaborating with the management team, the PCC ensures nursing care aligns with established protocols and serves as a resource for staff, enhancing the patient experience.

Key Responsibilities:

  • Collaborate with a multidisciplinary team to coordinate, assess, customize, and deliver superior patient-centered care.
  • Facilitate admission and discharge processes.
  • Work alongside subject matter experts and leadership to enhance outcomes and service quality.
  • Provide coaching, feedback, and training to elevate performance and engagement.
  • Assist in staff scheduling.
Qualifications

To be considered for this role, candidates must possess:

  • Registered Nurse (RN) license.
  • Bachelor's Degree in Nursing.
  • Basic Cardiac Life Support (BCLS) certification, or CPR, to be obtained within 30 days of employment.
  • Advanced Cardiac Life Support (ACLS) certification to be obtained within 60 months of employment.
