Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Case Manager. As a key member of our care team, you will be responsible for performing patient assessments, collaborating with the care team to develop and implement a plan of care, and providing education to patients, families, and caregivers.
Responsibilities- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to care center policies.
- Record pain/symptom management changes/outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with clinical manager and care team.
- Perform on-call responsibilities and on-call services to patients/families as assigned.
- One year of nursing experience.
- Current RN license, specific to the state(s) you are assigned to work.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Attractive pay: $80,000 - $85,000.
- Mileage and cell phone reimbursement.
- Benefits: first of month after hire.
- Territory: Guilford County.
