About the Role

We're seeking a skilled RN Care Manager to join our team at Mercy. As a key member of our interdisciplinary team, you'll play a vital role in assessing, planning, and coordinating patient care from admission to discharge.

Key Responsibilities
  • Assess patients' health needs and develop individualized care plans
  • Coordinate services to optimize health outcomes
  • Communicate effectively with patients, families, and healthcare providers
  • Advocate for patients' needs and promote health education
  • Facilitate smooth transitions of care across healthcare settings
Requirements
  • Graduate of an accredited School of Nursing
  • 1 year of acute care hospital setting experience
  • 2 years of acute care hospital setting experience
  • Care Management or Utilization Management experience
