Under the guidance of the Director of Family Support & Advocacy, the Licensed Practical Nurse (LPN) will play a crucial role in assessing, planning, and monitoring the health requirements of individuals, collaborating closely with the assigned Registered Nurse (RN). The nurse is an integral member of the multidisciplinary team within the Adult Foster Care Program and works directly with the Director of the Adult Foster Care Program.

In accordance with the regulations set forth by the Director of Advocacy & Family Support, the licensed practical nurses may:
  • Conduct and coordinate all necessary clinical assessments and evaluations;
  • Continuously develop and review each member's AFC care plan;
  • Oversee AFC caregivers in collaboration with the care manager;
  • Review the Primary Care Provider (PCP) Summary Form;
  • Monitor the health status of all members to ensure appropriate AFC services are delivered;
  • Report any changes in health status to the member's PCP;
  • Review AFC caregiver logs at least every 30 days;
  • Perform on-site visits with each member at the designated setting as per regulations;
  • Document progress notes for each on-site visit or encounter, or more frequently as needed;
  • Submit semiannual health-status reports to the member's PCP;
  • Plan and implement discharges and transitions from the AFC program;
  • Conduct orientation for each AFC caregiver prior to the commencement of personal care;
  • Provide ongoing training to AFC caregivers regarding health and aging.
Qualifications

  1. Licensed Practical Nurse with a minimum of four years' experience, including two years working with individuals with intellectual and developmental disabilities or the elderly. Must hold a current license in the State of Massachusetts.
  2. Possess a valid driver's license and have access to an insured vehicle.
