BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Moonachie, New Jersey.

Job Summary

We are looking for a compassionate and experienced Physical Therapist to provide high-quality care to our clients in their homes. As a Physical Therapist with BAYADA, you will have the opportunity to work with a diverse range of clients, from adults to geriatric patients, and make a meaningful difference in their lives.

Responsibilities
  • Conduct home visits to assess and treat clients with physical therapy needs.
  • Develop and implement individualized care plans to meet the unique needs of each client.
  • Provide manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and mobility.
  • Collaborate with nursing staff to ensure seamless care coordination and communication.
  • Educate clients and their families on rehabilitative care and activities necessary for optimal health and safety.
  • Accurately document client progress and care management using our state-of-the-art touch pad tablet.
Requirements
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
