Position Overview


The role of the Physical Therapist involves evaluating, treating, documenting, and facilitating care for residents experiencing functional impairments due to various health conditions. This position requires participation in interdisciplinary team meetings and collaboration with the Director of Nursing Service to develop policies aimed at preventing declines in resident health.


Key Responsibilities

  • Conduct evaluations and document findings for referred residents in line with facility policies.
  • Deliver appropriate treatments to help residents achieve or maintain optimal functional levels within their physical capabilities.
  • Recommend and/or provide suitable treatments, preventive measures, and adaptive equipment to avert declines in residents' functional abilities.
  • Maintain accurate and timely resident reports as mandated by regulatory agencies and third-party payers.
  • Adhere to infection control protocols.
  • Ensure the safety of residents under supervision.
  • Complete assigned duties related to Minimum Data Set assessments.
  • Document evaluations, treatment plans, MDS assessments, resident care plans, and progress notes promptly.
  • Engage in resident care conferences, interdisciplinary meetings, and rehabilitation discussions as scheduled.
  • Contribute to initiatives aimed at enhancing resident care concerning Quality Indicators.
  • Perform Quality Assurance duties as assigned.
  • Collaborate effectively with residents, their representatives, facility staff, physicians, consultants, and ancillary service providers.
  • Comply with all facility policies and procedures.
  • Follow safety protocols established by the facility.
  • Adhere to guidelines outlined in the employee handbook.
  • Respect residents' rights as per facility policies.
  • Uphold the Corporate Compliance Program Code of Conduct and related policies.
  • Participate in Corporate Compliance Program training as required.
  • Comply with anti-discrimination and sexual harassment policies.
  • Safeguard the confidentiality and security of all resident and facility information.
  • Maintain a professional appearance by arriving at work in clean, neat attire.
  • Demonstrate dependability and punctuality by adhering to the work schedule.

Qualifications


To succeed in this role, candidates must be capable of performing each essential physical and intellectual duty as outlined. The following qualifications are necessary:


  • Excellent verbal and written communication skills, along with strong organizational abilities. Proficiency in reading, analyzing, and interpreting medical information. Ability to communicate effectively with residents, their families, and other healthcare professionals.
  • Strong reasoning skills to identify problems, gather data, establish facts, and draw conclusions. Familiarity with software applications such as Word and Excel.
  • Doctorate in Physical Therapy.
  • Current Physical Therapist Licensure.
