Valid driver's license is required. American Heart Association Basic Life Support certification is necessary. Current state license to practice physical therapy is also required.

Job Details
As a Homecare Physical Therapist at Phelps Health Hospital, you will develop a plan of care for each patient assigned after evaluation/assessment to meet the patients and physicians goals of care, specific to the patient. You may be assigned home health or hospice patients. You will report to the Hospice Interdisciplinary Team regarding care and treatment of other team members as assigned to develop an interdisciplinary plan of care for each patient. You will also supervise PTA as assigned.

Education and Experience
Bachelor's degree in Physical Therapy from an American Physical Therapy Association and an American Medical Association accredited program is required. A minimum of one year experience in an acute care hospital setting is also required. Additionally, a minimum of one year experience in a rehab facility or equivalent serving the adult population is required. Six months home care experience is preferred. Computer skills are also preferred.

Schedule Information
Phelps can call off one shift per 30 days without penalty. GSA hours are not guaranteed the week of Orientation. You are guaranteed set hours per week, except during holidays where the unit is closed.
