Social Worker

The Social Worker at Campbell County Health plays a vital part in providing bereavement support and necessary services to Home Health, Hospice, and Bereavement clients. This role involves establishing and providing bereavement support, including individual support sessions and bereavement support groups for patients and families, as well as organizing and assisting with community bereavement events.
Key Responsibilities
- Establish and provide bereavement support, including individual support sessions and bereavement support groups for patients and families.
- Organize and assist with community bereavement events.
- Consult and provide social services for Home Health and Hospice patients, actively participating on the Hospice IDT team.
- Consult patients and families on services provided by Hospice and Home Health.
- Perform initial social service evaluation and psychosocial and spiritual assessment of patients.
- Implement a social service plan of care for Hospice and/or Home Health patients and update the plan as indicated by patient and family needs.
- Facilitate assistance and coordinate services for patients and families, including use of community resources, to assist with financial needs, Advance Directives, funeral planning, etc.
- Facilitate use of ancillary disciplines appropriate to the patient's plan of care.
- Document interventions and interactions with patients and families in the medical record.
- Comply with the hospital's corporate compliance program, including the Code of Conduct, laws and regulations, and hospital policies and procedures.
Requirements
- Bachelor's degree in social work from an institution accredited by the Council on Social Work Education, or a bachelor's degree in psychology, sociology, or other field related to social work.
- No licensure required.
- Certifications required, as per Cardiopulmonary Resuscitation Certification Policy and Certifications/Education Requirements Policy.
- Preferred experience.
Knowledge, Skills, and Abilities
- Ability to maintain continuing education requirements and all certifications and licenses as required.
- Knowledge of social work.
- Able to assess, appraise, evaluate, and understand complex and technical health concepts and client functioning.
- Ability to define problems, collect data, establish facts, and draw conclusions.
- Ability to apply concepts of fractions, percentages, ratios, proportions to practical situations.
- Ability to perform multiple tasks simultaneously with frequent interruptions and under stressful conditions.
- Analytical and reasoning skills with a high level of mental concentration and memory.
- Excellent verbal communication skills, ability to mediate and interact with people from diverse populations.
- Ability to interact positively with patients, patient families, physicians, and staff to effectively care for patients.
- Computer knowledge, including the ability to enter material into electronic records.
- Ability to demonstrate knowledge of developmental stages and apply developmental theories/concepts when planning and implementing care for the appropriate age of the patient.
- Must demonstrate and maintain current knowledge and skill in the following age groups: infant (neonate to age 1), pediatric (age 1-11), adolescent, adult, and geriatric (age 65 and older).
