MDS Coordinator

We are seeking a skilled MDS Coordinator to join our Clinical Services team at Lifespace Communities. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality care to our residents.
Key Responsibilities- Participate in resident care planning and coordination, ensuring accurate and timely documentation of resident conditions and nursing needs.
- Lead and direct licensed practical nurses and certified nursing aides, providing guidance and oversight to ensure exceptional care delivery.
- Initiate and verify physician orders, administer medications, and provide treatments according to established protocols.
- Facilitate communication with families regarding changes in medications and resident care plans.
- Develop and implement protocols to ensure care plans are continually reviewed and updated to reflect resident individuality.
- Nursing diploma or associate degree in nursing from an accredited program (Bachelor's degree preferred).
- One year of experience working in a long-term care facility.
- Registered Nurse license and other state-required certifications.
