Key Responsibilities

We are seeking a highly motivated and experienced Nurse Practitioner - Clinical Leadership to join our team at Dimensions Home Health Care. In this role, you will be responsible for providing clinical leadership and guidance to our nursing staff, ensuring the delivery of high-quality care to our residents.

About the Role:

  • You will be responsible for developing and implementing individualized care plans, working closely with our interdisciplinary team to ensure seamless delivery of services.
  • As a leader on our team, you will supervise and mentor nursing staff, providing guidance and support to ensure that all aspects of resident care are met efficiently and effectively.
  • You will also be responsible for participating in quality improvement initiatives, identifying areas for improvement and implementing strategies to enhance resident outcomes.

Requirements:

  • A Master's degree in Nursing and relevant experience in a healthcare leadership role.
  • Excellent leadership and communication skills, with the ability to motivate and inspire team members.
  • Strong strategic thinking and problem-solving skills, with a commitment to delivering high-quality care and improving resident outcomes.
