Position OverviewThe role involves managing cases for high-risk adults enrolled in Medicaid programs, aiming to enhance health outcomes and minimize unnecessary healthcare visits.
Key Responsibilities:- Deliver comprehensive case management to patients facing medical and psychosocial complexities, ensuring improved adherence to treatment plans and reduced hospital admissions.
- Act as a key liaison for mental health care strategies developed in collaboration with community partners.
- Identify and facilitate access to a variety of public and private resources at local, state, and national levels.
- Collaborate in the development and execution of care plans with medical professionals, patients, and community partners to ensure clarity and compliance.
- Maintain documentation in accordance with departmental and professional standards.
- 1-3 years of experience working with patients who have complex medical and psychosocial needs, including those with mental health or co-occurring disorders.
- At least 1 year of experience in case management.

- Licensure from multiple states is required, with credentials from the primary state needed prior to employment.
- Licensure as a Clinical Social Worker or a Master's Social Worker in Oregon within 6 months of hire.
- Licensure as a Social Worker Associate-Independent Clinical or equivalent in Washington within 6 months of hire.
- Valid Driver's License required where applicable.
- National Provider Identifier required upon hire.
- Basic Life Support certification within 3 months of hire.
- In-depth knowledge of psychosocial diagnostic categories and evidence-based treatment modalities.
- Exceptional communication and collaboration skills with diverse populations.
- Understanding of co-occurring mental health and substance use disorders.
- Current awareness of community mental health programs and resources.
- Familiarity with a broad range of community social services.
- Willingness to learn and utilize various computer applications.
- 2-5 years of case management experience with medically and socially complex patients.
- Recent experience in a managed care organization.
