Job Summary:

Piedmont HealthCare is seeking a skilled Social Worker MSW PRN to join our team. The ideal candidate will have a Master's degree from an accredited social work program and a strong background in assessing, evaluating, and coordinating community services.

Responsibilities:

  • Assess and evaluate patients' psychosocial needs and develop comprehensive transitional care plans.
  • Advocate for services to meet patients' complex needs and coordinate with the care team to provide solutions for complex cases.
  • Conduct high-risk assessments and identify patients at risk of readmission.
  • Coordinate with Care Managers daily and provide post-discharge follow-up to ensure continuity of care.
  • Participate in PHC readmission management initiatives and identify quality variances and/or barriers to discharge.

Qualifications:

  • Master's degree from an accredited social work program required.
  • None.
  • None.

Preferred Qualifications:

  • Prefer a minimum of two (2) years of experience in an acute or post-acute setting.
  • Licensed Master Social Worker (LMSW) in the state of Georgia preferred.
