MDS Coordinator

The ideal candidate will have a strong background in healthcare and experience with medical records documentation. As an MDS Coordinator, you will be responsible for scheduling and completing MDS assessments, assisting with care plan scheduling and completion, and transmitting MDS assessments to CMS. You will also be responsible for overseeing and monitoring MDS documentation and charting requirements, educating staff on MDS processes, and assisting with data collection for audits. **Job Responsibilities:**

Scheduling and completing MDS assessments

Assisting with care plan scheduling and completion

Transferring MDS assessments to CMS

Overseeing and monitoring MDS documentation and charting requirements

Educating staff on MDS processes

Assisting with data collection for audits

**Requirements:**

Licensed LPN or RN in the state of Indiana

Strong background in healthcare and experience with medical records documentation

Excellent communication and organizational skills

Ability to work independently and as part of a team
