POSITION SUMMARY:
The Care Services Coordinator plays a crucial role in supporting families impacted by ALS.
This position focuses on optimizing the functional capabilities of patients and caregivers, serving as a vital link between healthcare professionals and community resources to guarantee prompt and effective care.
KEY RESPONSIBILITIES:
Program Implementation and Evaluation:
- Execute care management initiatives aimed at improving the quality of life for patients.
- Oversee client caseloads, formulate care strategies, and coordinate clinic assistance.
- Assess client requirements, create care plans, and advocate for essential services.
- Collaborate with ALS Treatment Centers to ensure seamless care transitions.
Support Group Facilitation:
- Organize and lead support group sessions for clients and their families.
- Arrange educational workshops and invite knowledgeable speakers.
Administrative Responsibilities:
- Uphold compliance with organizational standards and maintain accurate client records.
- Generate reports, correspondence, and memos utilizing Microsoft Office applications.
Community Education and Outreach:
- Implement educational initiatives for the community.
- Advocate for ALS Association events and programs.
- Maintain professionalism and engage effectively with diverse stakeholders.
QUALIFICATIONS:
- Bachelor’s degree in public health, social work, nursing, or a related discipline.
- At least 3 years of experience in healthcare or community service.
- Familiarity with Medicare, Medicaid, and case management is preferred.
- Proficient in Microsoft Office and database management.
- Excellent communication and interpersonal abilities.
COMPENSATION:
The ALS Association offers a competitive salary for this role, with an hourly rate of $27.99. The organization provides a comprehensive benefits package, including healthcare coverage, a 401(k) plan, disability insurance, and paid time off.
