POSITION SUMMARY:
The Care Services Coordinator plays a crucial role in supporting families impacted by ALS.

This position focuses on optimizing the functional capabilities of patients and caregivers, serving as a vital link between healthcare professionals and community resources to guarantee prompt and effective care.



KEY RESPONSIBILITIES:

Program Implementation and Evaluation:

  • Execute care management initiatives aimed at improving the quality of life for patients.
  • Oversee client caseloads, formulate care strategies, and coordinate clinic assistance.
  • Assess client requirements, create care plans, and advocate for essential services.
  • Collaborate with ALS Treatment Centers to ensure seamless care transitions.

Support Group Facilitation:

  • Organize and lead support group sessions for clients and their families.
  • Arrange educational workshops and invite knowledgeable speakers.

Administrative Responsibilities:

  • Uphold compliance with organizational standards and maintain accurate client records.
  • Generate reports, correspondence, and memos utilizing Microsoft Office applications.

Community Education and Outreach:

  • Implement educational initiatives for the community.
  • Advocate for ALS Association events and programs.
  • Maintain professionalism and engage effectively with diverse stakeholders.

QUALIFICATIONS:

  • Bachelor’s degree in public health, social work, nursing, or a related discipline.
  • At least 3 years of experience in healthcare or community service.
  • Familiarity with Medicare, Medicaid, and case management is preferred.
  • Proficient in Microsoft Office and database management.
  • Excellent communication and interpersonal abilities.

COMPENSATION:
The ALS Association offers a competitive salary for this role, with an hourly rate of $27.99. The organization provides a comprehensive benefits package, including healthcare coverage, a 401(k) plan, disability insurance, and paid time off.
