Job Summary:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, NY. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to deliver exceptional patient outcomes.

Responsibilities:

* Provide daily support to the unit leadership and respond promptly to resident and family needs within the LPN scope of practice
* Use the nursing process, judgment, and skills to set priorities and deliver quality direct resident care
* Perform and administer standard nursing treatments and medications, observing residents for desired or unwanted effects
* Communicate and collaborate with the care team to revise and update the Nursing Care Plan or any pertinent findings
* Monitor and document vitals, treatments administered, and residents' responses
* Prepare, organize, oversee, monitor, and evaluate the delivery of care for resident CNA assignments and activities

Requirements:

* Graduate of an approved school of Nursing
* Licensed and currently registered in good standing in New York as an LPN
* Dedicated to providing extraordinary, clinically sound, and compassionate care
* Must possess a spirit of cooperation, caring, and enthusiasm to create an atmosphere conducive to rehabilitation and optimal resident health
* Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents
* Have excellent communication, customer service, and organization skills
* Work well under pressure and willing to work harmoniously with all staff
