We are seeking a skilled Medical Social Worker to join our team at CenterWell Home Health. As a Medical Social Worker, you will play a vital role in providing comprehensive care to our patients and their families.
Key Responsibilities- Evaluate and develop personalized plans of care to meet the emotional and social needs of patients.
- Assess patients' social and emotional states, identifying areas where support is needed.
- Provide guidance and supervision to Social Worker Assistants as required.
- Collaborate with healthcare teams to develop and implement effective care plans.
- Identify and connect patients with community resources to support their recovery and well-being.
- Document patient progress and maintain accurate records.
- Participate in discharge planning and care coordination activities.
- Supervise and evaluate the performance of Social Work Assistants.
- Masters or doctoral degree from a school of social work accredited by the Council on Social Work Education.
- Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice.
- Knowledge of and ability to assist with discharge planning needs and obtain community resources.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
