As an Occupational Therapist, you will play a vital role in our interdisciplinary team, collaborating with schedulers, managers, and other healthcare professionals to provide optimal patient experiences and meet patient needs.

Key Responsibilities:

  • Evaluate patient data to identify functional impairments and develop individually designed programs of occupational therapy treatment.
  • Manage caseloads, scheduling therapy revisits and reassessments as needed, and confer with patients, family members, caregivers, physicians, and multidisciplinary team members to plan, implement, and assess intervention programs.
  • Make referrals to physical therapy, speech therapy, nursing, social work, home health aide, and hospice services to meet patient needs.
  • Review physician referrals and patient medical records to determine diagnosis and occupational therapy treatment required.
  • Administer ADL/IADL retraining, therapeutic exercises, therapeutic activities, range of motion treatment, home safety training, and cognitive retraining to improve function, increase tolerance for meaningful activity, and improve patient quality of life.
  • Evaluate treatment effects at various stages and adjust treatments to achieve maximum benefit.
  • Instruct patients, family members, and caregivers in treatment procedures to increase carry-through with the plan of care and improve patient outcomes.
  • Recommend appropriate equipment and home modifications to improve patients' function in the home environment and improve caregivers' ability to help patients.
  • Perform medicine reconciliation as part of a multidisciplinary team, monitor patient vital signs, and communicate findings to team members and physicians.
  • Respond appropriately to changes in patient vital signs and discharge patients from occupational therapy when goals or projected outcomes have been attained or the patient is no longer homebound.
