About the Role

We are seeking a compassionate and experienced Licensed Practical Nurse to join our team at Adoration Home Health and Hospice. As a LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver individualized care plans.

Responsibilities
  • Provide 1:1 care for clients, following their plan of care and maintaining their dignity and independence.
  • Monitor clients' vital statistics and report any abnormal findings to the Case Manager and Physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and assess the results.
  • Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
  • Ensure infection control policies are maintained at all times, reporting any possible communicable diseases or infections.
  • Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
  • Monitor incidents involving individuals, ensuring incident reports are completed timely and accurately.
Requirements
  • LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
