Job Title: PACE Occupational Therapist

Care Partners is seeking a skilled Occupational Therapist to join our PACE team. As an Occupational Therapist, you will work with older adults to help them maintain their independence and quality of life.

Job Summary:

The PACE Occupational Therapist will work under the supervision of the PACE Center Manager and operate independently in applying occupational therapy knowledge and skills. Primary responsibilities include conducting assessments, care planning, collaborating with the PACE Interdisciplinary Team, performing home safety assessments, and managing a comprehensive therapy program alongside the Physical Therapist.

Key Responsibilities:
  • Conduct initial and ongoing occupational therapy assessments to evaluate participant needs, preferences, and goals. Develop measurable goals and design rehabilitation treatment plans.
  • Actively participate as a member of the IDT for comprehensive participant care planning.
  • Monitor input from participants, caregivers, and team members, updating the IDT promptly on any changes in medical status.
  • Oversee skilled and therapeutic exercises in the PACE Rehabilitation Gym, ensuring they meet industry and organizational standards.
  • Provide education to participants and families on rehabilitation needs and interventions.
  • Work collaboratively with other therapists and primary care staff to deliver comprehensive care.
  • Document all treatment and interventions promptly and accurately in participants' electronic health records.
  • Ensure respect for participants' rights and address ethical issues appropriately.
  • Plan and implement home-based interventions, including installation of adaptive equipment for participant safety. Conduct home safety assessments and other visits as necessary.
  • Supervise program aides and other staff implementing occupational therapy treatments or maintenance programs.
  • Participate in quality improvement processes, staff training, and departmental meetings.
  • Provide care in person, via telehealth, or via telephone as indicated by participant needs.
  • Support the enrollment of new participants in the PACE program.
  • Perform additional duties as assigned by the manager or supervisor.
  • May provide after-hours on-call coverage to support PACE participants outside regular business hours.
Qualifications:
  • Graduate of an accredited Occupational Therapy program (accredited by the American Occupational Therapy Association).
  • Valid California Occupational Therapy license in good standing.
  • Valid CA driver's license with a clean driving record for the past five years.
  • Current BLS (Basic Life Support) and First Aid certification.
  • Knowledge of proper body mechanics to avoid work-related injuries and ability to educate others.
Preferred Qualifications:
  • At least one year of clinical experience with older adults.
  • Advanced practice in Dysphagia strongly preferred.
  • Knowledge of therapy equipment and technology used in rehabilitation for older adults, including aerobic and strength conditioning equipment and mechanical assist devices.
  • Minimum of two years of clinical experience working as an Occupational Therapist with adults.
Language Requirements:
  • Fluency in English (reading, writing, speaking).
  • Fluency in Chinese (Cantonese and/or Mandarin) is preferred.
  • Proficiency in additional languages is an asset.
