Bishop Gadsden Episcopal Retirement, a leading life care retirement community in Charleston, South Carolina, is seeking an experienced Physical Therapist Assistant to join its rehabilitation team.

As a Senior Physical Rehabilitation Specialist, you will be responsible for implementing the plan of care as established by the Physical Therapist and Physician in a professional and competent manner.

A typical day for this role involves:

  • Screening all residents in the facility on a quarterly basis to identify those who have experienced declines in their functional abilities and determining if physical therapy intervention is indicated.
  • Carrying out and identifying the need for modification of the treatment program according to the plan of care in consultation with the Registered Physical Therapist.
  • Monitoring and reporting resident performance and progress.
  • Assisting the Registered Physical Therapist in re-assessment of patients and determining the need for continued services or discontinuation of services based on patient progress.
  • Assisting the Registered Physical Therapist in discharge planning.
  • Educating staff, family, and caregivers as indicated.
  • Completing all required documentation according to guidelines specified in the Documentation Manual.
  • Coordinating in-services for facility staff as needed to ensure safe and effective carry over of Maintenance/Restorative Programs and overall care of residents.

To be successful in this role, you must have:

  • An Associate Degree from an accredited Licensed Physical Therapy Assistant program.
  • Meet all certification and licensure requirements established by the state and national board associations.
  • Familiarity with Rehab Optima, Ultipro, Healthcare Source, MedBridge, PlanSource, RightTrack, PCC, and other computer software programs.
  • An active South Carolina license.
  • Active CPR certification.
  • Excellent organizational and time management skills.
  • The ability to stand, stoop, bend, reach, and lift up to 50 pounds.
  • Demonstrate professionalism, required assessment abilities, and clinical skills.
  • The ability to communicate clearly and effectively both verbally and written with residents, families, visitors, and Bishop Gadsden employees.
  • The ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
