Job SummaryWe are seeking a skilled Nurse Practitioner/Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, identifying potential health risks and developing personalized care plans to improve their overall well-being.
Responsibilities- Conduct thorough health assessments, including medical history, medication adherence, and social environment evaluation
- Develop and implement personalized care plans to address identified health risks
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination
- Provide education and support to patients and their caregivers on healthy lifestyle choices and disease management
- Document all assessments and care plans in our electronic medical record system
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patients' homes and work in a variety of settings
