As a Care Manager at Sunrise, you will play a vital role in providing the highest degree of quality care and services to a consistent group of seniors and their families in our assisted living and reminiscence neighborhoods.
Responsibilities- Build meaningful relationships with seniors and their families, providing assistance with activities of daily living and attending to individual care needs.
- Notify management of changes in condition and recommend adjustments in the level of care and service.
- Assist residents in life skills and other life-enriching activities as indicated on their individual profile.
- Participate in the development of Individualized Service Plans (ISPs) and monthly updates.
To be successful in this role, you will need:
- Dedication to serving seniors with excellent customer service skills.
- A positive attitude and flexibility to perform various duties in service to residents.
- High School diploma/GED accepted and may be required per state regulations.
- In states where applicable, must maintain certifications.
- Ability to make choices, decisions, and act in the resident's best interest.
