Responsibilities:
  • Provide high-quality, patient-centered care to meet the unique needs of the patient.
  • Develop and implement a personalized care plan in collaboration with the patient's physician and healthcare team.
  • Administer medications, perform medical procedures, and provide wound care as needed.
  • Monitor the patient's condition, report any changes or concerns to the physician, and make adjustments to the care plan as necessary.
  • Communicate effectively with the patient, family, and healthcare team to ensure seamless care coordination.
Requirements:
  • Current Illinois RN or LPN license.
  • Minimum of one year of nursing experience.
  • Private duty experience preferred, but not required.
  • Current CPR certification.
  • Recent TB test (within the last year).
  • Yearly physical document.
  • Excellent communication, organizational, and interpersonal skills.
  • Skillful, accurate, and timely clinical documentation and supply management.
  • Physically able to perform the job duties required for this position.
