Friday, Saturday, Sunday, & Monday
Working Hours: 8am-6pm CST

$39-$43/hour

Key Responsibilities:
Facilitates effective communication and coordination among all members of the care team.
Supports all patients throughout the care continuum by serving as a patient advocate and navigator.
Engages with providers and patients regularly through a telehealth platform, receiving consistent updates on patient status, including vital signs and assessments.
Monitors and prioritizes high-risk patients, ensuring they have access to necessary services tailored to their needs.
Maintains comprehensive documentation of provider and patient interactions, including visits, intake, interventions, patient concerns, and other elements of the patient's individualized care plan.
Oversees clinical and operational workflows.
Delivers prompt, courteous, and exceptional service to both internal and external customers at all times.
Collaborates with patients and the multidisciplinary team to evaluate and document measurable healthcare objectives. Communicates any patient-related environmental barriers to adherence to the care plan to the appropriate providers.
Shares discharge information with other clinical departments or care team members.
Identifies trends and challenges within the healthcare system and incorporates solutions for system issues, including feedback from patients, families, and physicians into an evolving process that enhances quality and satisfaction with the care experience.
Establishes and maintains collaborative professional relationships with physicians, medical directors, clinicians, and the broader community to develop and implement effective cross-continuum care management.
Qualifications:
Associate of Science in Nursing.
Current RN license specific to the state of employment.
Three to five years of experience in acute nursing.
Preferred Qualifications:
Bachelor of Science in Nursing.
Care Coordination and Transition Management (C.C.C.T.M) certification.
