Care Manager II

Job Summary:
The Care Manager II is a critical role within the healthcare team, responsible for ensuring seamless patient progression through the continuum of care. This position requires collaboration with patients, families, physicians, and multidisciplinary team members to develop and implement effective care plans.
Key Responsibilities:
- Coordinate patient care progression throughout the continuum of care, ensuring efficient and cost-effective delivery of services.
- Assess patient needs and develop comprehensive care plans, taking into account social, emotional, and financial factors.
- Collaborate with medical staff, nursing staff, and ancillary staff to eliminate barriers to efficient delivery of care.
- Interview patients and families to gather information about their needs and preferences.
- Develop and implement discharge plans, ensuring a smooth transition to post-acute care settings.
- Act as a patient advocate, negotiating with payors and vendors to secure necessary resources.
- Document patient information and care plans in accordance with regulatory and departmental guidelines.
Requirements:
- Graduate of an accredited school of nursing (BSN preferred) or Masters Degree in Social Work (MSW) required.
- Two or more years of clinical experience, with one year in the acute care setting preferred.
- RN or LMSW licensure in the state of employment required.
- Certification in Case Management preferred.
