Job Description

As a Medical Healthcare Specialist, you will provide comprehensive healthcare services to patients with complex and chronic care needs. Your responsibilities will include:

  1. Conducting initial and annual face-to-face patient visits in home and clinic settings
  2. Performing health risk assessments, including medical history review, symptom evaluation, and physical examinations
  3. Developing care plans for chronic disease management
  4. Collaborating with a multidisciplinary team to ensure quality patient care

Requirements

To be successful in this role, you must have:

  • Valid Nurse Practitioner or Physician Assistant license
  • Valid DEA License
  • Advanced practice clinician experience preferred (new grads considered with at least 1 year of clinical patient care experience)
  • Strong clinical assessment and diagnostic skills
  • Excellent communication and teamwork abilities
