About the Role

We are seeking a skilled and compassionate RN Case Manager to join our team at Ohio Living Home Health and Hospice. As a key member of our home health team, you will be responsible for providing high-quality patient care and coordination services to patients in their homes throughout several counties.

Key Responsibilities
  • Provide ongoing assessment and nursing care to patients in their homes, including developing care plans and providing treatment as needed.
  • Work closely with patients and their families to ensure seamless care coordination and communication.
  • Maintain accurate patient records through the use of mobile devices and electronic medical records.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for patients.
Requirements
  • Current Ohio nursing license and at least one year of acute care or medical-surgical experience.
  • Experience in a Medicare-certified home health agency and knowledge of electronic medical records (EMR) is preferred.
  • Valid driver's license, insurance, and acceptable driving record.
