About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Agape Care Group. As a Hospice and Palliative Care Specialist, you will play a vital role in providing high-quality care to patients with serious illnesses.

Key Responsibilities
  • Deliver palliative care and hospice services to patients in their homes or other care settings
  • Collaborate with a multidisciplinary team to develop and implement patient care plans
  • Provide education and support to patients and their families
  • Conduct regular assessments and evaluations to ensure patient needs are met
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
  • 2 years experience within the last 5 years in chronic disease management/long-term care
  • Hospice background preferred
  • Reliable transportation
  • Ability to sit, stand, bend, move intermittently and lift at least 25 lbs
