About the Role

Hillcrest Commons Extended Care is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our extended care facility.

Key Responsibilities
  • Provide Direct Patient Care: Administer medications, perform various nursing tasks, and provide emotional support to patients.
  • Document Patient Information: Accurately document patient care, including nursing interventions, patient responses, and progress towards goals.
  • Collaborate with Healthcare Team: Communicate effectively with RNs, physicians, and other healthcare professionals to ensure seamless patient care.
  • Teach Patients and Families: Educate patients and their families on health information, medication management, and self-care techniques.
  • Perform Administrative Duties: Complete medical forms, reports, and evaluations as necessary, and maintain accurate records.
Requirements
  • Current LPN licensure in the state of practice
  • Experience in extended care or skilled nursing setting
  • Strong communication and interpersonal skills
  • Ability to work effectively in a team environment
