The estimated annual salary for this position is $50,000-$55,000, reflecting the cost of labor across multiple U.S. geographic markets.

Job Description
In this role, you will perform initial nursing assessments on new admissions, complete and revise total plans of care, and maintain continuity of patient care by collaborating with staff. Regularly communicate patient progress to the clinical manager, physician, and care team. Develop and maintain individualized patient care progress records with accuracy and timeliness. Educate families on patient condition, needs, and care. Make referrals to other hospice disciplines as needed or document rationale for not doing so. Perform on-call responsibilities and services to patients/families as assigned.

Required Skills and Qualifications
To be successful in this role, you must have a current RN license specific to the state(s) you are assigned to work, one+ year of nursing experience, current CPR certification, valid driver's license, reliable transportation, and liability insurance.
