Compassionate Care Provider

We are seeking a skilled and compassionate Physical Therapist to provide home health services to patients in the Peninsula region. This role requires a licensed professional who can develop and implement effective treatment plans to improve patient outcomes.
Responsibilities- Conduct initial and ongoing clinical assessments to identify patient needs and determine level of functioning.
- Develop and implement individualized treatment plans to meet patient goals.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.
- Bachelor of Science in Physical Therapy required.
- Two years of experience as a Physical Therapist in an acute care or rehabilitation setting highly preferred.
The estimated salary for this position is $83,000 - $110,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including health insurance, retirement plan, and paid time off.
