About the Position:
We are seeking a compassionate and skilled Physical Therapist to join our team at Northwell Health, providing personalized care to patients in their homes. This role offers a unique opportunity to work collaboratively with multidisciplinary professionals, drive positive outcomes, and make a lasting impact on the lives of our patients and families.
Key Responsibilities:
- Evaluate and treat patients with complex needs, developing individualized treatment plans that address their specific goals and objectives.
- Maintain accurate and detailed medical records, using EHRs and adhering to regulatory requirements, to ensure continuity of care.
- Collaborate with healthcare teams, communicating effectively and addressing concerns in a timely and empathetic manner.
- Participate in quality improvement initiatives, contributing to the development of policies, procedures, and best practices that enhance patient care and outcomes.
- Assume responsibility for own professional growth and development, staying current with industry trends, research, and emerging technologies.
Qualifications:
- A Master's Degree in Physical Therapy, or equivalent combination of education and experience, is required.
- CURRENT licensure to practice as a Physical Therapist in New York State, plus specialized certifications as needed.
- A minimum of 1-3 years of experience in home healthcare settings is preferred.
Compensation and Benefits:
The estimated annual salary range for this position is $51,830-$79,300, based on factors such as location, experience, and qualifications.
