Clinical Pharmacist Job Summary

The Clinical Pharmacist will provide comprehensive pharmaceutical care to patients by delivering clinically oriented pharmacy services in the inpatient, outpatient areas, and/or correctional facility.

Key Responsibilities:
  • Monitor drug therapy by analyzing patients' drug therapy with respect to potential problems, laboratory data, and special procedures.
  • Provide consultation on the selection of drug therapy and appropriate dosing regimens, determination of therapeutic endpoints, total parenteral nutrition, clinical pharmacokinetics, and intravenous therapy.
  • Participate in patient care rounds, attending, and grand rounds.
  • Monitor and evaluate patients' drug therapy.
  • Prepare patient admission medication histories on selected patients using accepted guidelines and document this information in the patient's permanent medical record.
  • Provide patient education and counseling on selected patients using accepted guidelines.
  • Participate in the education and training of pharmacy staff and pharmacy trainees.
  • Initiate and/or participate in pharmacy and clinical drug investigations.
  • Conduct drug utilization evaluations and participate in patient continuous quality improvement projects.
  • Serve as the department of pharmacy representative on various committees throughout the institution.
  • Prepare literature evaluations for new drugs being requested through the Pharmacy and Therapeutics committee.
  • Contribute written information for the Pharmacy Newsletter.
  • Remain current with medication and pharmaceutical literature.
  • Maintain state of Illinois continued education (CE) hours at an annual minimum of 15 C.E contact hours per year.
  • Record and submit workload statistics/clinical interventions as required by departmental administration.
  • Participate in the management of medical emergencies, i.e., disasters, cardiopulmonary resuscitation.
  • Receive verbal medication, or medication-related orders from authorized practitioners and reduce them to writing on a Physicians' Order Sheet (POS) or input into Cerner, in accordance with established policies and procedures.
  • Monitor, detect, and report adverse drug reactions (ADR's) and medication administration errors in accordance with established policies and procedures.
  • Assist in proper functioning of the automated drug distribution system, including but not limited to monthly inspections of clinical areas.
  • Audit patient care areas for outdated medications, proper handling of pharmaceuticals, including controlled substances, and emergency supplies.
  • Perform job functions of a staff pharmacist, when necessary.
  • Maintain the work area in a clean and orderly manner.
  • Perform other duties as assigned.
Requirements:
  • Must possess a Doctor of Pharmacy Degree.
  • Must be licensed as a Pharmacist in the State of Illinois in good standing.
  • Must have successfully completed a PGY1 (Clinical Pharmacy Residency).
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, etc.) and clinical applications.
Preferred Qualifications:
  • One (1) year of comparable clinical pharmacy experience.
  • Prior Mental Health experience.
  • Board certification.
  • PGY2 Pharmacy Residency in Psychiatry.
Knowledge, Skills, Abilities, and Other Characteristics:
  • Knowledge of hospital and pharmacy policies, procedures, and techniques, Federal and State Laws, and pharmaceutical entities available in the U.S. market and their proper nomenclatures.
  • Knowledge of Pharmacokinetics, drug and poison information system, Drug Utilization Evaluation (DUE), and Adverse Drug Reaction (ADR) reporting systems.
  • Dexterity to manipulate drugs and containers and labeling.
  • Skill to compound and package medicines.
  • Skill to use computer keyboard for prescriptions entry and to use typewriter, typing twenty-five (25) word per minute accurately.
  • Ability and skill to communicate with a wide variety of individuals or groups.
  • Ability and skill to maintain accurate records, perform within reasonably established standards of accuracy within the department.
  • Ability to withstand the computer typing, prolonged periods of staring at the screen, and troubleshooting hardware.
  • Physical dexterity and mental alertness.
  • Ability to work under tension and at times under crowded conditions.
  • Ability to walk and stand prolonged periods.
  • Ability to maintain composure when dealing with patients who may be irritable and in discomfort.
  • Ability to lift heavy objects; eyesight must be correctable to provide visual acuity of reading accurately.
  • Ability to work various shifts.
  • Willingness and ability to work other stations, and perform additional work assigned by management.
