Job Description

Challenge Unlimited Inc is seeking a skilled Case Manager/QIDP to join our team. As a key member of our staff, you will be responsible for providing person-centered services to individuals with disabilities, helping them achieve their goals and maximize their independence.

  • Key Responsibilities:
  • Develop and implement person-centered plans to meet the unique needs of each client.
  • Provide instruction and feedback to clients to help them achieve their goals.
  • Evaluate and document client progress, ensuring compliance with regulatory standards.
  • Assist with the development of integration opportunities for clients, including employment and training programs.
  • Supervise staff and provide guidance and support as needed.

Requirements:

  • Bachelor's degree in a human service-related field or a Registered Nurse.
  • One year of experience working with individuals with developmental disabilities.
  • Certifications and licenses: CPR, 1st Aid, Crisis Prevention Institute (CPI), QIDP, and Direct Support Person (DSP) training provided by the company.
  • Computer skills: Proficient with Microsoft Word, Excel, and Outlook.
