Job Summary

SSM Health is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring patients receive the best possible care and support throughout their hospital stay and beyond.

Key Responsibilities
  • Assess patients' discharge planning needs and develop personalized plans to ensure a smooth transition to the next level of care.
  • Collaborate with interdisciplinary teams to identify and address potential barriers to discharge, including social determinants of health and access to community resources.
  • Utilize evidence-based practices and clinical expertise to optimize patient outcomes and reduce readmissions.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance the patient experience and improve clinical outcomes.
Requirements
  • Graduate of an accredited nursing program.
  • Current RN licensure in the state of practice.
  • At least 3 years of experience in acute care or case management.
  • Strong clinical skills and knowledge of discharge planning processes.
  • Excellent communication and interpersonal skills.
