Job Summary

We are seeking a highly skilled Family Nurse Practitioner to join our team at Evernorth Health Services. As a Family Nurse Practitioner, you will provide advanced nursing care to patients with acute, critical, and complex chronic health conditions.

Key Responsibilities
  • Perform initial patient screenings, patient history, and physical examinations to determine care plans and treat health conditions.
  • Update patient health records and ensure records are accurate at each patient appointment, ensuring continuity of care and collaboration with other healthcare professionals.
  • Analyze test results and imaging reports to determine the need for follow-up appointments and/or further treatment options.
  • Provide ongoing care information, counseling, and guidance for patients with critical conditions.
  • Perform chronic health assessments to determine health conditions and develop treatment plans, including ordering labs, imaging, referrals to specialty, and/or prescribing medication.
  • Perform differential medical diagnosis, including ordering, conducting, and interpreting diagnostic laboratory tests, including radiology studies and EKGs.
  • Order pharmacologic and non-pharmacologic treatments in the direct management of acute and chronic illness.
  • Respond to onsite emergencies until EMS can arrive.
  • Use medical training and expertise to respond to the rapidly changing physiologic status of patients.
  • Assess response to therapeutic interventions and recommend continued or changed treatment plans.
  • Direct and clinically supervise the work of nurses and other members of the care team.
  • Act in accordance with legal and ethical professional responsibilities, including patient management, documentation, and advance directives.
Qualifications
  • Board Certified Family Nurse Practitioner required.
  • Graduate of Certified Nurse Practitioner Program (Master's, post-masters, or doctoral preparation).
  • Active and unrestricted AZ license required.
  • Prefer more than 3 years of experience working as an NP in a primary care setting.
  • Pharmacy and DEA prescription privileges required.
  • BLS certification required with ability to utilize an AED, ACLS, and PALS preferred and required for Urgent Care.
  • Excellent team building skills.
  • Good computer skills using MS Office Products (Word/Excel) and Electronic Health Records systems.
  • Ability to communicate effectively with both internal and external customers.
